Birth Registrations
|
The
Township of North Dundas verifies all births that take place at
the Winchester District Memorial Hospital in North Dundas.
It is the responsibility of the parent to complete the Statement
of Live Birth that is provided to them by the Hospital and return
it to the township office located at 636 St. Lawrence St.,
Winchester, ON K0C 2K0 with the $20.00 administration
fee. Once we verify that the information on your statement
and the notice of live birth given to us by the hospital is the
same, it is sent to the Office of the Registrar General for
registration by the province. After you receive confirmation
of your baby's registration from the province, you can apply for
your baby's birth certificate. For more information on
having a baby, please visit the
Life Events section of the Ontario Government web site . |
